Triagem de transtornos alimentares em pacientes com diabetes em serviço público

Resumo

Objetivo: Investigar a prevalência de transtornos alimentares em pacientes com diabetes mellitus tipo 1 e a correlação com depressão, ansiedade e estresse. Método: Estudo observacional transversal. A amostra foi composta por 111 pacientes com diabetes mellitus tipo 1, avaliados por meio de questionários estruturados, incluindo o Questionário de Padrões Alimentares e de Peso-5, o questionário Sick Control One Stone Fat Food, e a Escala de Depressão, Ansiedade e Estresse. As variáveis foram analisadas por meio de estatística descritiva. Associações entre variáveis categóricas foram testadas com Qui-quadrado (incluindo simulação de Monte Carlo quando necessário) e variáveis quantitativas com teste t de Student. Variáveis com p < 0,20 na análise univariada foram incluídas em modelo multivariado de regressão logística, utilizando método backward, considerando p < 0,05 como significativo. Resultado: Os resultados revelaram que 37,8% dos participantes apresentaram transtornos alimentares de acordo com o teste Sick Control One Stone Fat Food, enquanto o teste Questionário de Padrões Alimentares e de Peso-5 identificou uma prevalência de 2,7%. Além disso, foi observada uma associação significativa entre a presença de transtornos alimentares e níveis elevados de índice de massa corporal, assim como entre transtornos alimentares e depressão. A análise dos sintomas de saúde mental mostrou prevalências de 22,5% para depressão, 21,6% para ansiedade, e 13,5% para estresse entre os pacientes. Conclusão:  Os achados indicam que pacientes com diabetes mellitus tipo 1 apresentam risco aumentado para transtornos alimentares, especialmente quando associados a maior índice de massa corporal e sintomas de depressão. Esses resultados reforçam a necessidade de estratégias de triagem precoce e acompanhamento integrado que incluam a saúde mental.
