Age and polypharmacy as risk factors for potential interactions of psychotropic drugs via CYP450

Abstract

The choice of an appropriate psychotropic agent requires knowledge of mechanism of action of the enzyme cytochrome P-450 system. The objectives of this study were to identify the sociodemographic predictors of elderly people with potential risks for serious interactions between psychotropic drugs via cytochrome P-450 attended at primary care and the prevalence of interactions. This is a cross-sectional study, carried out from 8/2014 to 8/2016, with elderly patients in an electronic medical record in public pharmacies. 1,603 elderly were investigated, with mean age of 74 years, mostly women (73.1%), of white race (80.6%) and unmarried (60%). Potential serious interactions were identified in 13.5% of the patients involving psychotropic medications and 29.4% involving psychotropic and non-psychotropic drugs. The polypharmacy was 65.0%, with a significant association with the presence of serious interactions between psychotropic medications (OR = 3.8, CI 1.7 - 8.3) and between psychotropic and non-psychotropic drugs (OR = 8.8, CI 4.5 – 17.3). Psychoanaleptics and psycholeptics also showed a significant relationship with the presence of potential serious interactions, (OR 1.5, CI 1.1 - 2.2, OR 1.9, CI 1.3-2.9), respectively. CYP3A4 and CYP2D6 enzymes were more frequent. It is concluded that patients between the sixth and eighth decade of life associated with polypharmacy have high risk for serious potential interactions between psychotropic drugs involving cytochrome P-450, with a high prevalence of serious interactions.
